DCMTK - DICOM Toolkit <BSD>: DCMTK is a collection of libraries and applications implementing large parts the DICOM standard.
Dicom3tools: Tools and libraries for handling offline files of DICOM 3 attributes, and conversion of proprietary formats to DICOM 3. Can handle older ACR/NEMA format data, and some proprietary versions of that such as SPI.

Grass Root DICOM <BSD>: GDCM is yet another C++ library dedicated to reading/parsing and writing DICOM medical files. GDCM stands as a short for "Grass roots DiCoM".
MINC(Medical Imaging NetCDF): MINC (Medical Imaging NetCDF) is a medical imaging data format and an associated set of tools and libraries.
BICPL(the McConnell Brain Imaging Centre Programming Library): BICPL is the BIC Programming Library, a set of functions for manipulating volumes, objects such as polygons and lines, some basic geometry, and some general programming utilities. It is based on top of the MNI Volume IO library, which is described in a separate document.
MITK(Medical Imaging Interaction Toolkit) / http://sf.net/projects/mitk: The Medical Imaging Interaction Toolkit (MITK) is a C++ class library which aims at supporting the development of leading-edge medical imaging software with a high degree of interaction.
ITK(the National Library of Medicine Insight Segmentation and Registration Toolkit): ITK is an open-source software toolkit for performing registration and segmentation. Segmentation is the process of identifying and classifying data found in a digitally sampled representation. Typically the sampled representation is an image acquired from such medical instrumentation as CT or MRI scanners. Registration is the task of aligning or developing correspondences between data. For example, in the medical environment, a CT scan may be aligned with a MRI scan in order to combine the information contained in both.
Qt4 Dicom Widget set <GPL/LGPL>: The Qt4 Dicom Widget set is extends Trolltech's Qt4 designer with additional widgets. They are 'components' to setup a DICOM viewer in some simple drag and drop operations. No coding needed.
Grassroots DICOM <BSD>: Grassroots DiCoM is a C++ library for DICOM medical files. It is automatically wrapped to python/C#/Java (using swig). It supports RAW,JPEG (lossy/lossless),J2K,JPEG-LS,RLE and deflated. It also comes with DICOM Part 3,6 & 7 of the standard as XML files.
HUG Papyrus toolkit <LGPL>: The Papyrus toolkit 3 is a library of C routines that greatly eases the process of reading and writting DICOM, DICOMDIR and Papyrus files. It is available for the Windows, Macintosh and Unix plateforms.

openDICOM.NET <GPL/LGPL>: openDICOM.NET - DICOM library, console tools, DICOM viewer/browser for Mono/.NET and Beagle Desktop Search Plugin. The project supports ACR-NEMA and DICOM file formats and provides transcoding to XML.
mDCM <LGPL>: mDCM is a .Net class library created with the goal of implementing large parts of the DICOM standard.
DICOM# <LGPL>: DICOM# open source project is intented to provide an OO class library for DICOM communication and will be developed purely by C# and running in .NET environment. DICOM# partially rewrites dcm4che open source project in C#.

ezDICOM <BSD>: ActiveX and CLX DICOM components のページでDICOM向けActiveXコントロール及びCLX(クリックス:Component Library for Cross platform)コンポーネントを公開している。
ソースコードはDelphi。Borland社から入手できるDelphi Personal Editionでバイナリは作成可能(らしい・・・)。また、サンプルプログラムにはVB.NETやC#ベースのものもあり。

PixelMed Java DICOM Toolkit: This is a stand-alone DICOM toolkit that implements code for reading and creating DICOM data, DICOM network and file support, a database of DICOM objects, support for display of directories, images, reports and spectra, and DICOM object validation.
dicom4j <GPL>: Dicom4J is a free, and open source DICOM implementation in Java. The library is mainly focused on Network communications.

DICOM.pm: DICOM.pm is a Perl library that allows Perl programs to read the headers of medical image files conforming to DICOM standards.
